Fertilität, Infertilität und Hypogonadismus des Mannes

Ziel dieses Forschungsschwerpunktes ist die endokrinologische und spermatologische Charakterisierung fertiler und infertiler Männer sowie die Therapie der männlichen Infertilität. Im Bereich der klinischen Krankenversorgung wurden mehrere Studien zur Bedeutung des männlichen Sexualhormons Testosteron für den Knochenstoffwechsel und die Regulation der Hormonsekretion vorgenommen Weitere Untersuchungen beschäftigten sich in Kooperation mit dem Institut für Klinische Chemie um die Erforschung von Auswirkungen einer Testosterontherapie auf den Fettstoffwechsel. In kontrollierten klinischen Studien untersuchten wir die pharmakokinetischen Auswirkungen von FSH auf die Inhibin B Produktion, um die physiologische Regulation der Inhibin B Sekretion zu überprüfen. Die Ergebnisse zeigten, daß neben der FSH-Sekretion die Spermatogenese der zweite wichtige Regulator der Inhibin B Produktion ist. In einem hyopgonadalen Patienten konnten wir eine Deletion eines Exons im LH-Rezeptor nachweisen. Diese führt zu einer Diskrimination der LH und hCG Wirkung. Eine Behandlung des Patienten mit hCG führte zu einer normalen Spermatogenese. Beim Weißbüschelaffen Callithrix jacchus fehlt Exon konstitutiv. Damit steht uns ein Tiermodell zur Verfügung, welches detaillierte Studien zur Interaktion von hCG undLH mit dem LH-Rezeptor ermöglicht. Zwei allelische Varianten des FSH-Rezeptors mit unterschiedlicher Affinität für FSH wurden identifiziert. Diese gentischen Varianten haben einen ganz erheblichen Einfluß auf die Stimulation des Ovars durch FSH bei Frauen, die im Rahmen einer assistierten Reproduktion behandelt werden. Mikrodeletionen des Y-Chromosoms sind eine wesentliche Ursache der männlichen Infertilität. Es wurden Grundlagenstudien sowohl zur Evolution und Mutation der Gene des Y-Chromosomes als auch zur hormonellen Regulation der Genexpression durchgeführt. Klinische Studien beschäftigten sich mit der Prävalenz von Mikrodeletionen des Y-Chromosomes. Das 1998 im Auftrag der Europäischen Akademie für Andrologie initiierte internationale Qualitätskontrollprogramm wurde fortgeführt und Richtlinien für die Durchführung von molekularbiologischen Untersuchungen auf Deletionen von Genen auf dem Y-Chromosom entworfen. Zur Zeit nehmen 57 Labore weltweit an dem von Prof. M. Simoni in unserem Institut organisierten Programm teil. Für dieses Programm wurde Frau Prof. M. Simoni mit dem Richard-Merten-Preis in Höhe von 15.000 DM ausgezeichnet.
